Leads on iPad research tools?

I'm looking at putting into place a more robust system for observing our visitors, and I'm trying to put together a data collection/management system. It would be great to take an iPad out on the floor and collect data digitally straight away, but I can't seem to find the right app. Any students out there with leads on great social research data collection technology?
